In the event that the interior or exterior of a home is damaged during delivery there are various responsibilities that fall on the customer and transportation service provider (TSP). In the case that the TSP causes property damage, this form will be presented upon discovery during the delivery. VERY IMPORTANT - Please notify the TSP within 7 calendar days of any damage that occurred during delivery. Regardless of whether or not it is noted on the form. The 7 days begins after the first day of delivery. Failure to notify the TSP within 7 calendar days can result of denial of your claim. Please see below for the full customer and TSP responsibilities.
