In the event that the interior or exterior of a home is damaged during pickup there are various responsibilities that fall on the customer and transportation service provider (TSP). In the case that the TSP causes property damage, this form will be presented upon discovery during the packout. VERY IMPORTANT - Please notify the TSP within 7 calendar days of any damage that occurred during pickup. Regardless of whether or not it is noted on the form.
